Your Rights & Responsibilities

This hospice complies with applicable Federal civil rights laws and does not discriminate on the basis of sex, race, color, religion, ancestry, national origin, sexual orientation, disability, medical condition, marital status, or registered domestic partner status. Patients and their representatives (if any), have the right to be informed of the patient’s rights in a language and manner the individual understands. As a Hospice provider, we protect and promote the exercise of these rights. 

Patient Rights at AVA Hospice Corp

As a patient, you have the right to:

- Exercise Your Rights: Fully engage with your rights as a hospice patient.
- Receive Respectful Treatment: Be treated with dignity, consideration, and respect, including privacy in your treatment and personal care, and have your belongings treated with care.
- Be Free from Abuse: Live free from verbal, mental, sexual, and physical abuse, as well as neglect and misappropriation of property.
- Receive Pain Management: Access effective pain management and symptom control.
- Participate in Your Care Plan: Be involved in the development of your hospice care plan.
- Refuse Treatment: Decline any care or treatment.
- Choose Your Physician: Select your attending physician.
- Access Your Records: Have a confidential clinical record and the right to access or request the release of information in accordance with HIPAA and our Notice of Privacy Practices.
- Receive Service Information: Get information about the services covered under the hospice benefit and the specific limitations of those services.
- Access Nursing Services: Reach nursing services 24/7 by calling our office.
- Know Your Care Supervisor: Be informed about the person supervising your hospice care and how to contact them during work hours and in emergencies.
- Make Healthcare Decisions: Make decisions regarding your healthcare, including acceptance or refusal of specific treatments and the use of Advance Directives. We will explain your options and provide a copy of our policy on Advance Directives.
- Receive Financial Information: Be orally and in writing informed about:
 - Expected payment amounts from Medicare, Medicaid, or other federal aid programs.
 - Charges for services not covered by these programs.
 - Any pre-care charges you may incur.
 - Changes in charges for services when they occur.
- Access Auxiliary Aids: Be informed about available auxiliary aids and language services and how to access them.
